Requirements 3.14
3.14 System and Information Integrity: the NIST SP 800-171 requirements
The 7 requirements of family 3.14, System and Information Integrity, in NIST SP 800-171 Rev 2, worth 31 points of a SPRS score under 32 CFR 170.24. For each: NIST's text, what it costs when not met, the objectives an assessor determines under NIST SP 800-171A, and how to meet it. To mark each one and work out your score, use the free calculator.
3.14.1 Flaw Remediation
Identify, report, and correct system flaws in a timely manner.
Points: 5 points.
Assessment objectives (NIST SP 800-171A)
- [a]The time within which to identify system flaws is specified.
- [b]System flaws are identified within the specified time frame.
- [c]The time within which to report system flaws is specified.
- [d]System flaws are reported within the specified time frame.
- [e]The time within which to correct system flaws is specified.
- [f]System flaws are corrected within the specified time frame.
How to meet it
Identify, report, and correct flaws on a defined timeline: subscribe to vendor and CISA advisories for your stack, run the monthly patch cycle across OS, applications, and firmware, and track exceptions with compensating controls. Measure patch compliance with your RMM or vulnerability scanner rather than assuming success. Evidence: the patch management policy with timelines, deployment reports, and the exception register.
3.14.2 Malicious Code Protection
Provide protection from malicious code at designated locations within organizational systems.
Points: 5 points.
Assessment objectives (NIST SP 800-171A)
- [a]Designated locations for malicious code protection are identified.
- [b]Protection from malicious code at designated locations is provided.
How to meet it
Deploy malicious code protection at the endpoints and the gateway: a modern EDR/antivirus (Defender for Endpoint, CrowdStrike, SentinelOne) on every workstation and server, plus email attachment/link scanning in your mail platform. Verify coverage: unprotected servers are the usual gap. Evidence: the EDR console showing full device coverage and email protection policy screenshots.
3.14.3 Security Alert Monitoring
Monitor system security alerts and advisories and take action in response.
Points: 5 points.
Assessment objectives (NIST SP 800-171A)
- [a]Response actions to system security alerts and advisories are identified.
- [b]System security alerts and advisories are monitored.
- [c]Actions in response to system security alerts and advisories are taken.
How to meet it
Monitor security alerts and advisories and act on them: designate who watches CISA alerts, vendor bulletins, and your EDR/SIEM notifications; define response actions (emergency patch, config change, threat hunt); and log what was done for significant advisories. A weekly advisory review habit with notes satisfies this at small scale. Evidence: the monitoring responsibility assignment and dated records of advisories reviewed and actions taken.
3.14.4 Malicious Code Updates
Update malicious code protection mechanisms when new releases are available.
Points: 5 points.
Assessment objectives (NIST SP 800-171A)
- [a]Malicious code protection mechanisms are updated when new releases are available.
How to meet it
Ensure malware protection updates automatically: cloud-managed EDR handles definitions continuously, but verify update status in the console and alert on agents that fall behind or go offline. Remediate stale agents as found. Evidence: console reports showing definition/agent currency across the fleet and the stale-agent alert configuration.
3.14.5 System & File Scanning
Perform periodic scans of organizational systems and real-time scans of files from external sources as files are downloaded, opened, or executed.
Points: 3 points.
Assessment objectives (NIST SP 800-171A)
- [a]The frequency for malicious code scans is defined.
- [b]Malicious code scans are performed with the defined frequency.
- [c]Real-time malicious code scans of files from external sources as files are downloaded, opened, or executed are performed.
How to meet it
Enable both real-time protection and periodic scanning: on-access scanning everywhere, scheduled full or quick scans on a defined cadence, and automatic scanning of removable media and downloaded files on connection/receipt. These are policy toggles in your EDR/AV product. Evidence: the protection policy export showing real-time, scheduled, and removable-media scan settings.
3.14.6 Inbound/Outbound Monitoring
Monitor organizational systems, including inbound and outbound communications traffic, to detect attacks and indicators of potential attacks.
Points: 5 points.
Assessment objectives (NIST SP 800-171A)
- [a]The system is monitored to detect attacks and indicators of potential attacks.
- [b]Inbound communications traffic is monitored to detect attacks and indicators of potential attacks.
- [c]Outbound communications traffic is monitored to detect attacks and indicators of potential attacks.
How to meet it
Monitor systems and traffic for attack indicators: enable your firewall's IDS/IPS, alert on suspicious identity events (impossible travel, brute force) via your SIEM or cloud identity protection, and use EDR behavioral detections at both the boundary and endpoints. A managed detection and response (MDR) service is a cost-effective way for small teams to get 24/7 coverage. Evidence: enabled detection rules, sample alerts with triage notes, and the MDR agreement if used.
3.14.7 Unauthorized Use Detection
Identify unauthorized use of organizational systems.
Points: 3 points.
Assessment objectives (NIST SP 800-171A)
- [a]Authorized use of the system is defined.
- [b]Unauthorized use of the system is identified.
How to meet it
Detect unauthorized use of your systems: alert on logons at unusual hours or from unusual locations, new device connections, and unauthorized software execution, using your SIEM, identity protection, and EDR detections; define in writing what "authorized use" means so violations are recognizable. Review these alerts as part of your regular log review. Evidence: the acceptable use definition, configured alert rules, and samples of investigated alerts.
